If you take regular medication please discuss it with your doctor who will arrange for your medical record to be updated. Details of what medciation you take are kept on computer under the terms of the Data Protection Act, 1998.
From 31st May 2011, the dispensary service for patients living outside Oswestry will no longer be available. This has been a difficult decision for the Caxton Surgery but we have now given up some of the space in our building to accomodate the Pharmacy @ Caxton, a joint venture with the former Lunts Pharmacy in Willow Street. This means that all patients will now need to collect their medication from a community pharmacy. Repeat prescriptions should be ordered in exactly the same way from the repeat prescription desk. It is just the supply of medication that will be different so you will need to let the receptionist know where you would like to collect your medication from.
To obtain your repeat prescription please give 48 hours notice.
You can leave a message on the answerphone by dialling 658519 giving your computer number, name and address and the name(s) of the medication(s). If you need to speak to a member of staff personally, you can dial direct on 659250.
Otherwise you can complete the order form attached to your previous prescription and hand it in to the receptionist or send it through the post.
You may also submit an order via the Repeat Prescription link below. You can simply open the first link and use the form provided via the Internet or, if you prefer, you can use the bottom link which is more secure but you need to be registered for the service. To make sure we give the best possible care, patients who receive regular medication from their doctor should have a drug review at least once a year. The back of the prescription listing your repeat medication includes the date when your review becomes due and need to see your doctor again. The doctor will need up to date information, and to make the process as simple as possible, it is important that you have an appointment with a health care assistant at least one week before you see your GP. Please tell the receptionist you are having a drug review and will need two appointments.
The health care assistant will up date your medical record with any relevant information such as: height, weight, blood pressure etc. and can take any routine blood tests so that the results will be available by the time you see the GP.
If you suffer from coronary heart disease or diabetes you should be aware that if you have not had your cholesterol checked within the last 12 months you will need a fasting cholesterol blood test. So please remember not to eat or drink anything for 12 hours before. (You can drink small amounts of clear fluids, including tea or coffee without milk.)
